A Life Reclaimed: Tackling Multiple Myeloma with Expertise and Precision

A cancer diagnosis often disrupts life in profound ways, bringing both physical and emotional challenges. It marks a journey filled with questions, fears, and the hope that the right specialist can provide answers. For many, it’s the expertise and compassion of their doctor that helps transform uncertainty into a clear plan for recovery.

This was the case for Govindappa, a 70-year-old man whose life took an unexpected turn when he began experiencing severe pain in his limbs and back, along with swelling near his right collarbone. With no prior health concerns or comorbidities, his symptoms were alarming and unexpected. Concerned about his rapidly deteriorating condition, his family sought the guidance of Dr. Manish Kumar, a highly regarded medical oncologist in Bangalore known for his personalized and methodical approach to cancer care.

“Govindappa’s case was challenging due to its advanced stage,” Dr. Kumar shares. “His X-rays revealed widespread lytic lesions, and elevated M-protein levels confirmed multiple myeloma, ISS III. Our priority was to manage the disease effectively while focusing on his pain relief and quality of life.”

Given Govindappa’s age and the aggressive nature of the cancer, Dr. Kumar crafted a treatment plan tailored to his condition. The VRd regimen—Bortezomib, Lenalidomide, and Dexamethasone—was chosen as the foundation for therapy. “With older patients, we always balance efficacy with tolerability,” Dr. Kumar shares. “It’s not just about treating the cancer but ensuring the patient can maintain their daily routine and overall well-being.”

The results were significant. Within weeks of starting treatment, Govindappa’s pain subsided, and his mobility improved. “He told me he could finally sleep through the night without waking up in pain,” Dr. Kumar recalls. “That’s the kind of feedback that reminds you why this work is so meaningful. Seeing a patient regain their independence is immensely rewarding.”

For younger patients with multiple myeloma, stem cell transplantation is often the gold standard following induction therapy. However, Govindappa’s age made him ineligible for this approach. Instead, his treatment plan included maintenance therapy to sustain remission after the initial four-month induction phase. “Maintenance therapy is essential for older patients,” Dr. Kumar explains. “It helps keep the disease under control while ensuring the patient’s quality of life remains intact.”

Dr. Kumar’s expertise extends far beyond clinical treatments. With over two decades of experience, he specializes in hematological cancers, including multiple myeloma, leukemia, and lymphoma, as well as solid tumors like breast and lung cancers. As a Senior Consultant at Prakriya Hospital, he integrates cutting-edge therapies such as immunotherapy, precision medicine, and bone marrow transplantation into his practice. “Oncology is advancing at an incredible pace,” Dr. Kumar notes. “Techniques like CAR-T cell therapy and bispecific antibodies are opening new doors for patients with even the most complex diagnoses.”

The broader field of oncology is also seeing exciting developments in treating multiple myeloma. Experimental therapies such as CAR-T cell therapy involve reprogramming a patient’s immune cells to target and destroy cancer cells, while bispecific antibodies are designed to engage the immune system in attacking malignant cells. Although these innovations are still evolving, they represent a promising future for patients worldwide. “Staying updated on these advancements ensures we are always prepared to provide the best possible care,” Dr. Kumar adds.

Long-term management of multiple myeloma is equally crucial. Regular follow-ups and adherence to treatment plans are essential for sustained outcomes. “One of the challenges we face is ensuring patients remain consistent with their therapy, especially when they start feeling better,” Dr. Kumar explains. “That’s where counseling and education play a key role in reinforcing the importance of every phase of treatment.”

Govindappa had lots of compliance issues as well. Once he started feeling better he exhibited reluctance for further treatment. The weekly chemo schedule was also a challenge for him which required counselling on every visit. This is a very important part of treatment to keep the patients motivated to stick to treatment plan to get best possible outcomes

Govindappa’s journey underscores the life-changing impact of timely diagnosis, expert guidance, and personalized care. His progress highlights the power of modern oncology in transforming even the most daunting diagnoses into manageable realities. With a steadfast commitment to advancing cancer care, Dr. Kumar continues to inspire hope for countless patients and their families. By leveraging cutting-edge treatments and a deeply personalized approach, he offers patients the chance to reclaim their lives, redefining what is possible through expert guidance and compassionate care.
